Very Short Central Incisors with Dark Shadows at the Gum

Diagnosis

Patient #8 didn't like the unnatural appearance of the back teeth and canines. This is possibly the most difficult cosmetic case I have attempted. I thought she wanted a better smile line with the teeth following the lower lip in a smile. I thought she wanted more rounded incisal edges

Treatment

The teeth were bleached but not as much as she wanted.

We prepared the four upper anterior teeth for 3/4 ceramic crowns and the premolars for porcelain veneers. We made the temporary. It was not what she wanted. The temporary was remade to be more acceptable. The temporaries were then used to make the final crowns.

Future Treatment

She continued to bleach the lower teeth to match the veneers. The gums were irritated from finishing the veneers. On a follow up appointment, we found some additional roughness. The case has been in about 8 or 9 years and I recently saw her and everything looked great.

The average life expectancy for all ceramic veneers and crowns is 7 to 9 years.
